Thank you for choosing Dunn Physical Therapy to participate in your rehab care! Our highly trained therapists have the experience necessary to address your rehab needs. Our goal is to provide treatment in a comfortable and caring environment so you can achieve the best rehabilitation results possible.
To simplify the registration process for all new patients, our office will mail intake forms before your appointment. Please complete these forms prior to arriving at your first appointment. We ask that all new patients arrive at least 15 minutes prior to their scheduled appointment time to complete the registration process. If you have not received an intake packet before your visit, we ask that you arrive at least 30 minutes prior to your scheduled appointment time. We request that you submit your insurance information prior to your first session so that, as a courtesy, we will be able to check your insurance benefits prior to your arrival.
You will also want to bring the following to your first appointment:
- Government issued ID
- Insurance Card
- Your copayment or payment towards your coinsurance or deductible
- Referral from your physician, if applicable; (This is required by insurances such as Medicare and Tricare).
- Completed patient intake forms
- Patients under the age of 18 must be accompanied by a parent or legal guardian.
- We recommend patients wear comfortable clothing and athletic shoes to their first appointment.
Your First Visit
Your first appointment will include an evaluation to assess your current status. At this time, your therapist will develop a plan of care specific to your needs and rehab goals. This initial appointment will also include education on your diagnosis and what to expect during your course of treatment. This is also a great time to address any questions about your condition and your plan of care.
Our practice is happy to accept most major insurances including Humana, Anthem, BCBS, Medicare and Tricare. To confirm our participation with a specific insurance plan, please contact our office. As a courtesy, our office will contact your insurance to confirm your quoted benefits. All copayments and payments toward your deductible and coinsurance are due at the time of service unless other arrangements have been made. Should you have a need to make other financial arrangements, please contact office prior to your appointment.
